1) HOW TO ENTER: Respondents are automatically entered into the
Ortho.com Insights
sweepstakes
("Sweepstakes") when they complete and submit the CRM Metrix survey online
between 07/21/05 and
07/01/06. Limit one entry per
respondent per survey/drawing. No responsibility is assumed by “Releases”, as defined below, and entrants waive all claims, for lost, late, illegible, incomplete, damaged which will be void, or for any problems or technical malfunction of any computer or network lines, computer equipment, software, failure of any email service or any combination thereof including without limitation any damage to entrant's or any other person's computer resulting from completion of the survey or participation in this promotion. Entries generated by script, macro or other automated means are void. All surveys and other entries become the property of CRM Metrix,

Sweepstakes is a violation of criminal and civil laws. Should such an attempt be made, Sponsor reserves right to prosecute violators to the fullest extent allowable by law.
2) PRIZE:
First Prize (1): $250
(U.S. dollars) Federal,
state, provincial, local
and any other applicable
taxes in an eligible
jurisdiction in
connection with prizes,
are the sole
responsibility of the
winner. No substitution
or transfer of prizes
permitted, except by
Sponsor due to prize
unavailability at time
of award. All prizes
will be awarded.
3) PRIZE DRAWING:
Winner(s) will be
selected in a random
drawing to be conducted
by CRM Metrix. Decisions
of Sponsor are final on
issues relating to this
Sweepstakes. The odds of
winning depend upon the
total number of eligible
entries received. The
drawing will take place
at CRM Metrix, New
Jersey offices on or
about 07/01/06.
Potential winners will
be notified by email. If
Sponsor is unable to
contact potential winner(s) by email, or
if any potential winner
fails to respond to the
prize notification
within 14 days of
issuance of notification
or if any potential
winner fails to comply
with any of the
requirements, an
alternate winner shall
be selected through
another random drawing
conducted by judges.
Allow 2-3 weeks after
verification of winner
status for the receipt
of prize by mail.
According to U.S.
Federal law, any prizes
awarded to a winner,
whose country of
residence for tax
purposes is other than
the U.S., will have 30%
of their winnings
withheld. [In order to
claim the withheld 30%,
winners will need to
file tax form 1042-S].
If, for any reason this
Sweepstakes is not
capable of running as
planned or infection by
computer virus, bugs,
tampering, unauthorized
intervention, fraud,
technical failures or
any other causes beyond
the control of Sponsor
corrupt or affect the
administration,
security, fairness,
integrity or proper
conduct of this
Sweepstakes, Sponsor
reserves the right at
its sole discretion to
disqualify any entrant
or to modify, terminate
or suspend, in whole or
in part, the
Sweepstakes.
4) ELIGIBILITY:
This Sweepstakes is open
to residents of the
United States
with internet access as
of 07/01/06 who are 13
years of age or older,
with the exception of
the ineligible
individuals described
below, who participated
in this online CRM
Metrix study. If an
individual under 18
years of age in the U.S
is selected as a winner,
their parent or legal
guardian is required to
give authorization
within the affidavit
form and the prize will
be issued in the name of
the parent or legal
guardian. For all other
potential winners, the
prize check will be
issued in the winner’s
name. This Sweepstakes
is not open to employees
of Sponsor, or its
affiliates, licensees,
parents, franchisees,
subsidiaries,
advertising or promotion
agencies or members of
the immediate family and
household, of each. In
these Rules, “immediate
family” means parents,
children, siblings and
spouse, regardless of
where they reside. By
entering, participants
without limitation,
release and hold
harmless Sponsor, and
its affiliates,
licensees, franchisees,
subsidiaries,
advertising and
promotion agencies and
their respective
employees, officers,
directors, shareholders,
successors and assigns
(collectively
“Releases”) from any and
all liability for any
injuries, loss or damage
of any kind in
connection with this
Sweepstakes or
acceptance or use of any
prize. Void where
prohibited. Any issues
concerning the validity,
interpretation or
enforceability of these
Official Rules, or the
rights and obligations
of entrants and Sponsor,
shall be governed by and
construed in accordance
with the laws of the
State of New York, USA,
without giving effect to
any conflict of law
rules which might find
